When you first start the game you’ll be prompted to either continue a saved world or begin a new one. New worlds are quite customizable in difficulty and setting. Once you make a world you can decide how many players there will be.
So now you start the game. You’re in your shelter, and on the left side of the screen you have options such as "wait" and "go outside". On the right side you have your food, water, meat, veggies. Keep a high supply of meat, water and fuel.
You go outside. Around you are the wasteland. You can see maybe a river, blast zones, and trees. You can fish in rivers and blast-zones or get water from them, from trees you can either get wood, hunt or cut them for fuel. With fuel your character uses less energy to get to places. Watch out though, there is a gauge on the top of your screen showing how exhausted your character is.

In my opinion, the game
was ok. It wasn't Fallout or Wasteland or nothing, but it’s satisfying enough. Give it a try.
